About LMCHD Health and Wellness Grant Program: LMCHD offers an annual grant program and a summer grant program.  LMCHD’s tax revenues support the Health and Wellness Grant Program which allocated funds to community health and well-being programs and services.  LMCHD uses revenues to award grants directed to health-related programs and services delivered by non-profit agencies serving residents of the Los Medanos Community Healthcare District.
